Dr. Hazard completed his undergraduate and doctorate at the University of Louisville and The United States Dental institue for orthodontics /facial orthopedics. He is a member of the Academy of General Dentistry, the American Orthodontic Society & has been in practice in the Louisville & Bardstown, KY areas with over 30 years’ experience.
