History

Established in 1998.

Mid-​State Health Center is an independent, non-​profit, Federally Qualified Community Health Center established in 1998 whose mission is to deliver comprehensive primary care services to its community regardless of ability to pay. Mid-​State operates with a clear and collective vision of providing high-​quality medical and behavioral health services to its geographically isolated, rural community. We are committed to the delivery of a coordinated, personalized care experience as part of an innovative community health model. Mid-​State fosters strong collaborative relationships with a wide variety of public and private entities within its service area and the State to support access to health care services in its effort to promote a healthier community and population.
