History
Established in 2004.
Angel On My Shoulder was created to fill a void in patient care. Often because of multiple responsibilities, distance and other family obligations, families and friends of patients under a physicians care are unable to be present as often as they’d like.
Conferences with physicians, nursing staff, and care givers are often held over the phone or missed altogether. Visits are sometimes hurried and the details of patient’s care and well being may be overlooked or difficult to gauge. Patient, family, and friends are often left with incomplete information as well as the worry that goes along with not being able to be involved in the patient’s care on an everyday basis.
That’s where we come in. Acting as an advocate of patient, families, and friends, Angel On My Shoulder will monitor the details of care and/or confer with physicians and care givers in order to assure quality of care. We’ll report our findings to you on a regular basis, and will be available to meet with you, the patients,

Business Owner
Carol is a Registered Nurse, Certified Case Manager, and Dementia Care Specialist with over 40 years of experience in Healthcare, and 20 years in Case Management.
As a case manager, she coordinates care between patients, families, and all healthcare providers involved. This is especially vital to patients, families, and friends that are overwhelmed. Her mission is to provide peace of mind by ensuring that the highest quality of care and services are consistently being provided.
Carol’s experience in Case Management includes acute hospital care, home care, care outside the home, rehabilitation, and long term care. She anticipates changes, develops a plan, and teaches her clients how to adapt their lifestyle to changes in their health.
She continues to demonstrated consistent, positive outcomes, and continually strives to improve her patient’s quality of life.
