At Forest Creek Family Dental Care we provide the highest quality in personal care and place a very special emphasis on reconstructive, restorative cosmetic dentistry. Our services include teeth whitening, veneers, dental implants, bonding, cosmetic dentistry, extractions, root canal therapy, and cosmetic fillings.

Your initial oral examination includes a visual examination, charting, periodontal probing, oral cancer screening, diagnosis and treatment recommendations. We will also take x-​rays, which includes the panoramic x-​ray for proper diagnosis of the anterior (front) and posterior (back) teeth as well as the bite-​wing x-​ray series for proper diagnosis of proximal (in between) decay of posterior teeth.

History

Established in 1980.

After twenty treasured years of delivering esteemed dental health care to the Indianapolis community, Dr. John F. Jansen announced his intentions to retire from private practice in March of 2011. He has chosen Dr. Jeremiah J. Frasier to purchase his practice due to his experience, education, and personality. Dr. Frasier is a graduate of the I.U. School of Dentistry. He is excited to meet existing patients and also wants to reach out to potential new patients for the practice.

Dr. Frasier, «J.J.» is a native of Muncie, Indiana and attended Ball State University. He is a graduate of the Indiana University School of Dentistry in 2002 and completed a Hospital Dentistry residency at the University of Washington in Seattle. He has practiced dentistry in Indianapolis as an associate since 2004.

He and his wife, Susan are proud parents of two young children — Lucy, age 4 and Simon, age 15 months. They are very happy to be assuming a dental practice which has maintained a highly family-​oriented focus for 20 years.
