Established in 2008.

Discovery Transitions is the outpatient component of The Discovery House. Individuals programs offer structured outpatient treatment to men and women. The program offers residents the support and treatment necessary to continue their commitment to recovery as they transition back into life free from drug or alcohol dependence.

Programs offer one on one sessions, sober coaching and advanced treatment options.
