BEWARE of Dr. Wayne Forman Ophthalmology, He saw my 12 year old daughter for migraines with post migraine vision changes. He did not dilate her eyes properly and was not able to see the optic nerve. He has been unresponsive to my repeated attempts to clear up the bill. I ended up seeing another doctor in the same practice who stated that her initial exam with Dr. Forman was not correct. She needed her eyes dilated 2 additional times to get a finding for her vision changes. I drove and hour to see these doctors and could not have been more disappointed. The only amount of the bill that I was disputing was $ 82.72 and he does not have the common courtesy to return a call or repeated letters to his office.
Beware of DR. FORMAN Ophthalmology!

Got my Flu shot.
Was told it would cost no more than $ 40.
Was billed $ 39.20 for the shot & $ 54 administrative fee.
This is outrageous!
The admin fee cost more than the shot & was NOT disclosed.
Called the billing office 3 times and got nowhere.
Get your immunizations anywhere else– not here.

Always a problem with these people. First instead of charing my card for the deductible for my wife and myself, which would be two charges, they charged it three times! Do they really need the money that bad.
Second, we had annual visits with the opthalmologist. However, if he was to rite our glasses prescription on a piece of paper and give it to us there would be an additional charge of $ 175 each.
This is the last time I use them. plenty of places to go in my area.
