Community Healthcare Network is a New York Presbyterian Healthcare System-​affiliated not-​for-​profit organization which provides more than 80,000 mostly low-​income and uninsured New Yorkers of all ages with an array of primary care, dental, nutrition, mental health and social services. Our network is made up of 11 federally qualified health centers throughout Brooklyn, the Bronx, Queens and Manhattan, along with a traveling medical van that offers free sexual health services to people in Manhattan and Queens.

We work in many communities where access to healthcare is limited. Our goal is to keep people out of the emergency room by introducing them to a preventive care model, rather than a crisis-​based one. CHN’s dedicated team of medical professionals works hard to make our healthcare services as convenient and accessible as possible to our patients. As a Federally Qualified Health Center, we never turn anyone away from care.

History

Established in 1981.

Our story begins in the late 1960’s, when a group of storefront clinics in NYC began dedicating themselves to offering family planning services. In 1981, to strengthen management and administration of these twelve community-​based health care centers, the Community Family Planning Council was established.

The clinics soon expanded to include primary care, mental health and social services. In 1998 the name was changed to Community Healthcare Network to better reflect our comprehensive array of services.

Since its inception, the agency has grown tremendously in response to such social crises as unequal distribution of medical care, adolescent pregnancy, AIDS, inner city poverty and homelessness.
