Justin S. I don’t understand your review and why you would give this place that you have never seen, a 5 star rating? To be clear, I am talking about Woodland Care in Reseda, Ca. It is a DUMP… Run!!! I have a friend currently there and I am praying that she can come home very soon as I would not put a dog in this place. All the reviews above are so true. I was there the other night and my friend sat for an hour and half in her urine after promises to come and take care of her. She pressed the call button several times and no one came. I went to the desk and asked question, what if she needs urgent attention? all i got was lip service. You could die in this place and no one would know. I felt depressed leaving her there.

If there were 0 stars that would be my review. My father was transfered from The hospital to this rehab facility We were very skeptical about sending him here due to all the bad reviews this place has. But unfortunately this is the only place his case manager could find that would take him. And we said well let’s give it a try and sadly all the bad reviews are true. My father arrived at 7pm and it now passed 9 pm and not even one nurse or worker had come to check up on him. My family and I had to go to the front and ask for information the Rn supervisor looked at us like he was bothered and said they PROBABLY and usually don’t start assessing the patient til the next day. We asked him if someone could stay with him because my father is legally blind and is to weak to do anything on his own because of side effects caused by chemo radiation and he also suffers from severe anxiety and they said because of policy they couldn’t allow it that they needed directors approval. nOw they supposedly Called the director and that the director said no that they would call us if anything went wrong at night. Mind you we emphasized my fathers legally blind and suffers from severe anxiety and we are trying to make there job much easier .I completely understand policy but if they made the effort to make people feel comfortable a safe it would make this whole situation much better especially when it’s his first night here…
So the rn or nurse assistant came in gave him a tv shot changed him after being here for more than 3 hrs left
10:45 pm and no one from the facility has come to explain anything about how this facility works nor have come to get our information down for the incase of an emergency. This place lacks work ethic and are very very unprofessional on how everything works here. So if your thinking of choosing this place look elsewhere.

I wish that I could give this facility 0 stars. It was truly the worst place that has ever cared for my mother. Fortunately, she only stayed here for 2 weeks, but in that time, her mental and physical health declined rapidly. She could never get assistance from any of the staff and they could not take care of her basic needs. Whenever we went there to get assistance, we noticed that the nurses were standing around talking. If you ask any of them any health-​related question, they have no idea what you’re talking about. I wanted to tell them to use google. A completely inept and incompetent staff. Also, PLEASE don’t deal with «social worker» Teresa. She is the most incompetent of them all and I am shocked that she’s able to work anywhere. Finally, they lost all my mother’s clothes so make sure to label everything. I’m reporting this facility to the state and any other reporting agency that I can find. As another reviewer stated, if you care about your loved one, do not put him/​her in this place!

Run, DO NOT walk away from this facility. Crowded, smelly, no parking for guests, residents and their visitors hanging around outside smoking, etc. Care level questionable, rooms crowded. Very few amenities for residents. Random old people sitting around in various stages of distress. Awful, just awful

My brother came in for a short rehab stay. I hadn’t been to the facility or known much about it before, but the service was good and he was able to return to his home after a few weeks. The staff treated him well while he was there, they had activities he could go to daily and he didn’t have to wait long for a nurse to help him. Overall it was a good experience and he would return if needed.

I have never visited this facility, but I think people may get this facility confused with another facility in Washington state. This is not the facility in Washington. I would also mention that health care facilities are regulated and strictly monitored by the state. A good resource to make sure that a facility is in compliance with all state laws is on the Medicare website. Unilocal is a great resource to review service driven industries like restaurants and hotels, but to solely base your decision on these reviews would be unwise. There may have been recent changes that have improved the business or problems may have been fixed that had come up in the past. That being said, do your research and contact the facility directly to address concerns that may come up.

This place sucks! My father in law was transferred here after suffering a stroke. He needed therapy to be able to walk again, and we were told he would be here until he walked with no help which they said would be about 2 months. Within a week the social worker told us he would be discharged, Even though he wasn’t walking yet. We asked the nurses, no one knew anything about him being discharged. The morning he was going to be discharged, my father in law called my husband like at 6:30am saying he had very bad chest and arm pain and that the nurse wouldn’t do anything. My husband rushed there and demanded they call 911 and they said it wasn’t necessary because they had given him Advil. My husband decided to call 911 himself. Turns out my father in law was having a heart attack! If it weren’t for him having his cell phone there to call us he would have probably died since no one there wanted to call 911. My father in law is currently in the hospital and will be transferred to a different rehabilitation center. Definitely not coming back to this place

This place is awful! They let my mother-​in-​law sit in her own urine for 2 hours! She has diabetes and had a low blood sugar attack and it took forever to get help. Then they sent someone in to help that didn’t speak English. All she needed was a couple jellybeans but the woman gave her the whole bag and, because she was so shaky, sticky jellybeans ended up all over her and the bed. We got her out of there! If you care about your family please find someplace else to take them.

Was sent here from hospital after a bone break /​surgery. It was not my desire to go to this facility. Preferred to go home with in-​home physical therapy. Turns out private insurance is not accepted by skilled nursing facilities closer to home.
Upon arrival, was hard to miss the shabby, dirty, dingy, dated, and sad condition of the property. There was visible dirt/​food remnants on the floors close to the dining hall and nurses’ station. Scores of elderly residents were parked in the halls, staring down anyone who passed. I’m not a senior yet, and felt out of my element.
It seemed apparent that the elder patients were largely being warehoused in this facility. There was supposed to be an «Express Recovery» section for people who’d had surgery or injuries which would require occupational /​physical therapy to get stronger in order to get home. I was supposed to be in that area, but was assigned to the adjacent ward as there wasn’t a bed in the desired ward.
I give the staff huge credit for carrying out their duty of caring for so many patients with dementia or other debilitating conditions. They mostly seemed to be caring and as responsive as they could be given the overall conditions and number of patients to care for; which was large. The place appeared to be close to capacity.
MediCal patients are stacked 3 to a room; tight quarters. Medicare or private insurance patients are in double rooms. Minimal privacy, and almost no place for visitors to sit. The rooms are so tight, that folding chairs are offered for visitors.
The doctor wanted me up and sitting in a chair during the day as much as possible, plus having PT and OT, however, the only chair safe for sitting would be a wheelchair. And the wheelchairs were junior-​sized to save space, so scant support for your legs, seat-​wise. Felt like perching on a stool; thus not comfortable.
The owners of the building should be required to stay in the place for a minimum of a month and not be allowed to leave at any time. They might then have a sense of what they’re offering to the public. Extremely poor conditions.
There is nothing in the place from this century — including hospital beds, TV’s, walkers, wheelchairs, PT /​OT equipment. Everything is old and shabby. The adjustable beds were partially broken and made harsh sounds when operated, like they were going to fall apart at any moment. Not at all comfortable, which meant the mattresses were old.
Blinds were broken and didn’t work properly. There was a full sized bathtub sitting in the middle of the room with no curtains around it, and no handicap bars — so it was a giant dust catcher?! Our friends who came to visit were appalled. My husband was so distraught about where I’d landed that he wasn’t able to sleep nights until I got released.
The walls were scraped up from contact by various means. Wall switch-​plates were haphazardly placed; some were askew so that you could see through to the wall underneath. TV cords were stretched dangerously long distances, dangling across the wall. A former flatscreen mount was left on the wall of the first room I was in, with an assortment of paint remnants across the wall. Nothing was finished or repaired.
I felt like I was in the «Worst Exotic Care Center». It was so depressing that I felt myself falling into a state of despair. I had to prove to the physical therapist that I was strong enough to go home. He agreed with me, but the doctor had to come in to sign off on the release. Except he doesn’t come on Wednesdays — because he’s out of Santa Ana! What would happen if there was a medical emergency, aside from calling 9−1−1? In essence, I was held captive to their schedule, because if I checked myself out against medical advise, my insurance wouldn’t pay for after care.
In two days, I had one PT evaluation, which lasted about 7 minutes, the one OT and one PT session the next day. This is intensive therapy? I don’t think so.
Food was actually better than the hospital I’d come from, which was a pleasant surprise.
Nights were rough. The patients started with their night terrors after sundown, and the shrieking didn’t cease until maybe 2 AM. Then the noise level went back up at 6:30 AM when the shift change began. Seemed like all the staff members were having a party; the noise level was substantial as they greeted each other and chitty-​chatted. And don’t request assistance at that time. I had to wait 90 minutes to get pain medication during shift change.
Also didn’t help when my roommate turned on a devotional program at loud volume at 6 AM. I was ready to scream from the accumulated circumstances of my surroundings. Do whatever you can to find another location for your loved ones!!!

My mother was admitted for Physical Therapy in May 2014.
During her stay she I had repeatedly requested to speak to her doctors.
The nurses sent «my messages to the Doctors».
I did NOT get to speak directly to her doctor’s.
Her medications caused complications.
She developed a nasty cough the last week of her stay. She also complained of having difficulty breathing.
Her nurse checked her O2 level & listened to her lungs.
I requested the doctor be notified AND a chest X Ray be done (this was Tuesday).
Wednesday, R.N. reported the Doctor had not responded.
Thursday, R.N. reported the X Ray was done but they were waiting for the reading.
Friday — R.N. reported the X Ray was «Negative».
Saturday — discharge day — my mother’s cough & complaints of SOB continued.
I requested to see the results. The R.N. looked through her chart and reported that the doctor NEVER responded to the original message & NO X Ray was done.
Discharged because Physical Therapy determined they couldn’t justify keeping her any longer because, «she can walk 400′ & Medicare wouldn’t pay for more time».
Upon her discharge she was UNABLE to walk.
She fell 6 times that first evening.
She can no longer feed herself. She was assigned a «Feeder»
She lost almost 15 lbs.
She is back in the hospital for FREQUENT FALLS.
The staff:
The Parking Valet was nice & helpful.
2 Nurses were nice. Most were dismissive.
1 Nurses Aid was FABULOUS.
The noise level is terrible.
When I toured the facility I commented on the noise. I was told it was due to the high level of interaction with the patients.
NO. It IS DUE TO the PARTY LIKE atmosphere of the staff.
One day the son (4 or 5 yrs old) of a staff member walked around «visiting» the nursing staff (for the four hours I was there). The staff was very animated & verbal with this child. Very distracting to the staff. But, a really cute child.
I WOULD NOT recommend this facility.

Come here with your own supplies. They have nothing to offer but staff who say yes ok we’re on it but never follow through. If you really love the person your considering bringing here, DON’T!!!

Prices are competitive. However, a few things, this establishment has no parking to speak of. Their own workers park on the street so imagine the parking you’ll be able to get. This also serves as a registered voters polling station so you can vote and visit family, or, spice it up, vice versa!
Security here is lacking. People jump the back fence of this establishment to get to the nearby liquor store. They don’t seem to do anything about it. Maybe install a camera or have a security guard check the area or build a better fence?
Their gardener shows up early on the weekends (7 – 8 am) with the loudest, most out of date equipment, disturbing everyone, they don’t address it. Sometimes the gardener is there both Saturday and Sunday.
Lastly, a good friend of mine who is an EMT/​Firefighter said that of all the places he’s worked with, the staff here is really subpar. He had harsher words, but I’ll leave it at that.
I would not personally ever use this facility, because even if prices are competitive, I want better for my people. Hope this helps
