Jacob Healthcare Center

January 2016, I had major back surgery and after talking to Dina who set up my stay, I was off a few days later to Jacob Healthcare Center for rehab. I found the staff that works there to be great, professional, helpful, caring and ready to help me in every way.
I was there for well over a month and they cleaned my room and others every day.
If I needed someone to help me I turned on the light and someone would be right there to help. I loved my own room and bathroom. It was nice not to worry about food for it was delivered three times a day to my room, you can also eat in the common area.
The nurses, CNA’s and therapists are special people always working hard to help you. I came away from Jacob Health care feeling that the staff from the front office to the people I worked with daily truly cared about me and the other patients and work hard to make sure everyone gets what they need and are happy.
Besides the nurses you have a Doctor to help you. While there my teeth were cleaned and a foot doctor saw me.
I was happy at Jacob and after learning to walk again because of the hard work of the therapists and staff, I am living the good life at home.
I would go back to Jacob Healthcare Center and I am happy to recommend it as a great professional Healthcare Center.

My mother in law passed away on November 1, 2015 due to an apparent Heart Attack, which she was on medication to prevent a heart attack while being cared for at Jacobs Healthcare center.
Within 24 hours of her death:
–coughing up blood
–swollen lymph nodes
–medications were stopped, which she was on to prevent a heart attack without cardiologist approval.
— wasn’t tended to after pressing emergency button for over an hour.
Some of the other issues that happened in the time she was there, which she was only there for one month:
–Not monitoring blood pressure which resulted to immediate transfer from routine cardiologist to ER.
Had to have feet elevated for over 5 hours due to low blood pressure. Jacobs didn’t monitor blood pressure according to doctors orders.
–Nurses claimed her trouble breathing was due to getting used to breathing, which in fact was pneumonia after We demanded an xray be completed on her chest.
–After she requested water to drink in the early afternoon, she never received the water until late morning the next day.
–She was being fed with 1.2 cal feeding tube, and nurse informed us that her feeding would start to be restricted including water with no explanation to us.
–Repeatedly given medications without informing patient or Renae which meds she was being given and at what appropriate times they are being given.
— Did not tend to patient after requesting assistance with over an hour going by (unless visitors were there)
— Ventilator alarm placed on silent. We came into room with Alarm on ventilator being on but was recently silenced with no nurse in the room.
— reset assistance light without asking patient what they needed.
–Got back ulcers and sores due to not turning her at appropriate times. Cleaned open sores with dry rags.
–Desk operator refused/​bypassed request to speak to administrator in regards to cause of death for Mother in law.
— Forged arbitration signatures for patient. Signatures on paperwork and administrator rushed through paperwork to avoid being questioned.
Everything was documented with all issues. Currently Public Health Licensing Bureau is investigating.

I just left my 3 week stay at Jacobs Health Care Center and I was very pleased with the medical care I received. I was hit by a car and broke my femur and needed surgery on my acl and lcl. After I left the hospital I spoke with Rachelle, the ²⁴⁄₇ administrator and she gave me my own room and bathroom for my upcoming stay at Jacobs. I was transferred and started physical and occupational therapy with a very wonderful team of therapists. They were very thorough and vested in my well being and physical care. I moved from a wheelchair to crutches in 3 weeks with the help of the staff. The nurses are great and delivered all of my medication on time without any problems and the CNA’s took care of all my communication needs with the doctors and nurses. My grandmother passed away Dec.27^th, 2015. I have no doubt that the problems she experienced here led to her fast decline. As soon as we left here in an ambulance we did our research and found Carmel Mountain Rehab which was the best rehab center I have experienced in San Diego. My grandma passed in their care, almost 2 months after her ordeal at this awful place. My family met with health and human services and they are investigating further.

this place is the most disgusting thing on earth. 1. the bathrooms don’t work.
2.they «supposedly» clean the rooms, which i wonder who taught those ladies how to clean. i can just imagine how their house looks.
3. you ring for the nurses and it’s more than 30 min before they show up, if they show up.
4. they yell at the patients and don’t treat them right.
5. never enough supply’s. the workers have to sometimes buy their own box of latex gloves (on of the nurses told me himself).
6. all they want to do is collect money from the patients. they don’t give a sh*t about the patients.
never never neeeevvveerr bring a loved one here.
this place has 1 star because they had two and they just got one taken away, what does that tell you??? this place should just be closed down for good.
the administration who runs this place is probably pocketing the money that’s why this place is such a dump with no supplies…

I’m a fairly new employee at jhcc, I’ve been here since October 2014. I can honestly say I love this place! We have many amazing staff members who truly care about our patients. Overall, I have had the pleasure of seeing many of our patients get well and go home. If u or a family member are looking for a place to recover and get stronger at your own pace, in a tranquil environment you should definitely come in for a tour and get a feel for our place. We’d love to show you around.

My father was placed at this facility after suffering brain injury. He got such bad care that we are back at UCSD for several infections my poor dad is trying to fight including a bed sore he now has because he wasn’t turned on schedule. Its so frustrating because we were getting ready to take him home and now we are set back. It’s such an unsafe place to be that I don’t know where to start from!
1– beds are so flimsy and unsafe that my mom stayed over night with him to make sure he wouldn’t fall off the bed.
2– the rooms do not get cleaned on a regular basis.
3– they always run out of clean sheets, towels, etc.
4– food trays are old and not thoroughly cleaned before serving their food.
5– foreign staff that do not speak or understand English.
6– «nurse”/patient ratio is about 30 beds to ONE nurse.
7– nursing staff is rude and unprofessional.
At first everyone is nice and try to meet with you but after that you’re on your own. It’s such a hard situation to try to find the best care for your loved family member, please do a lot of research but DO NOT GO HERE!

They’re taking great care of my dad. The CNAs work tirelessly to make sure all the guests are comfortable, especially Kim and Tiffany in the evenings. I’m lucky enough to spend a lot of time there and see them treat everyone well, even guests without family present. Don’t worry that they aren’t taking care of your family member.
They’re sanitary and clean, patient and polite. Can’t really ask for anything more.

I wouldn’t recommend this place to look after my fish, seriously. They almost killed my grandma after their on staff MD prescribed wrong medication to her which knocked her out completely. I wish it had ended there but once she was out no one noticed for THREE days that she had not gotten up, eaten or drank anything. She was in full blown kidney failure. I could go on and on but I’ll cut to it. When we first started our search we stupidly dismissed the Unilocal reviews, using the excuse that no one is happy to have a relative in a care facility. We dismissed the
rating because this place came recommended by a friend of a friend. Walking in the front door, Jacobs doesn’t look all that bad, right? WRONG!!! We took advantage of the time my grandma was back in the hospital and found a new place for her, Carmel Mountain Rehab. It is completely different from Jacobs. The nurses actually CARE, if the call light is on they come right away, they watch my grandma and have taken the lead on her healthcare, constantly monitoring and getting her the care she needs. NOTHING like this place. We feel 100% confident in Carmel Mountain rehab, nothing like Jacobs. At Jacobs you are lucky if they come in 20 mins once the light is on. The staff will not know your loved one from a fly on the wall. They short cut everything. We were actually told by a nurse on their staff that she had major concerns about this place. What’s crazy is there was a CNA training class being held here one day, now that I think about it I should try and find out who is in charge of that because NO CNA should be trained here. it’s bad enough to have to put your loved one in a care facility but DO NOT settle, go up the road a bit and get actual healthcare. No one at this place knows what they are doing, from the CNA’s to the administrator. We have filed a complaint with the appropriate people and we will continue to follow up on it. Get your act together Jacobs!

This place should have no stars and should be shut down. RUN from here if you are looking for a skilled nursing facility. As someone else mentioned, my grandmother had to be transported to the hospital by ambulance because of THEIR lack of care. The doctor prescribed wrong medication, she was not monitored for food and drink intake for 3 days+ (we don’t know how long because there were no records filed by nurses — which is ILLEGAL!) She was in kidney failure and could not advocate for herself, I was the one who found her in a semi comatose state. THEN we requested the doctor to start an IV because she was severely dehydrated at 10 a.m. and that did not happen until 9 p.m. Once in the ER, I found out it was placed incorrectly. This place does not hire professionals, they don’t train staff appropriately and you can tell the problems start from the top up. We never once received a phone call about a change in her health status (which is required by state law) in the 2 weeks she was there. The CNA staff is sub par and they do not have the skills or heart to work with elderly people. These were the BIG problems, I can also tell you it takes 25 – 30 min. to get a CNA when the call light is turned on, we brought food to my grandma and it was left for 3+ days out. These are all oversights by staff and again ILLEGAL. There are standards that HAVE to be met. We didn’t know better. Please please please look elsewhere. We have filed a report with Heath and Human Services.

I have the inside scoop… this place is garbage… trust me… this place is all about perception not quality. trust me I know how this place truly runs. You have been fooled if u think different, and in that case the best part about this facility is the sales department, selling you BS…

It’s a matter of time before this facility gets sued.
It is very dirty, and smells awful a lot of the time. I honestly dread going here to visit my grandmother just because of the stench.
We had an uneasy feeling today so we requested that they call an ambulance to bring her to a hospital. They refused!!! They kept arguing until we had to take matters into our own hands.
They said she was fine, but when she got to the hospital, she actually DOES have complications! She has a UTI and an aneurism!!!
Way to go. Wow. Very disappointed.

The only place I’ve been into that caregivers dont use wipes! Yes thats right, they just used an improvised pad (blue pad) and wet to wipe the residents. How gross right? Caregivers dont use walkie talkies making them no one to rely on in case of emrgency but yell for help, although there is a call light but how about if its unreachable?! Not a good place to work.

OK. I have a unique perspective on Jacob Health Care. I was asked to produce a 3 – 4 minute video to promote their facility in late December, 2012. This video will be uploaded to the Jacob Health Care website soon. I really had no preconceived ideas on the place going in. From the outside it looks somewhat average. But I have to say that after spending literally an entire day inside the facility shooting a promotional video, I was blown away. The entire staff is incredibly caring and genuinely love working here. I spoke with many patients and their families and I kept hearing the same story: How everyone is treated like family. The place is kept extremely clean… almost obsessively so. The bedrooms have hardwood floors and many of the rooms are private. If I had a relative that needed assisted living or sub-​acute care, I wouldn’t hesitate to have them admitted here… it’s that nice. I usually don’t give a review on a client I’ve produced a video for, but I felt in this case… and considering the category… it warranted it. Definitely go check this place out!
