Valley House Rehabilitation Center

The second time in our American life we use services of Valley Rehabilitation House
Center (991 Clyde Avenue, Santa Clara, California 9505) for our parents. At first time for my husband’s father, who was living in Valley House for 4 yrs until 2003.
Now my mother moved there in Nov 2014.
It is always not an easy step for us to make this new arrangement working for our
parents when they became sick and helpless. We cannot take care of them full time and we have to choose the place for them to live.
This time my mother was transferred to the Valley Rehabilitation House from O’Connor Hospital.
We didn’t have any doubts that it was the right place for her.
Valley House is highly professional place. She was placed to the Dementia Unit, and the management of this Unit is great.
Workers on all level here are caring and responsible. The rooms are clean.
Units 3 and 4 are closed Units for dementia patients. They can go for a walk to the garden only.
Often we see musicians, singers, magicians in the activity rooms.
The most impressive was the visit of a few beautiful big dogs whom everybody who wanted could pet.
Bingo is an everyday life.
And whenever I come I see the smile on the faces of the nurses and
the housekeeping staff.
It is not easy to get used to the new place, but you know that your old mother
or father are taken care of and the level of this care is high.

Simply amazing care! Exceeded all our expectations! My mom has very advanced Alzheimer’s. When caring for her at home was no longer possible, we placed her to Valley House in their long term care unit about 1 year ago. It was a very difficult decision to place her in an «institution». However, it was the best thing we’ve done for her, and at this point I don’t think of Valley House as an «institution».
First, about staff– main reason for five stars. The staff is simply amazing! So caring and warm, they seem like they truly care about their patients/​clients. The multidisciplinary team is involved, and keeps us updated. The unit manager Manya is just unbelievable, she knows all her clients by first name, takes time to get to know the families, and is always available. Everyone is incredibly supportive, warm and caring. They are very patient with residents, and manage to keep smiling. It makes a huge difference, and gives the family piece of mind. My mom is always clean, safe, and comfortable.
Secondly, about the facility. The rooms are very small, some are double, some are triple. The space in the room is very– very limited. They basically have hospital beds, curtains between the beds, small bedside tables, small wardrobe for clothes, and small TV mounted to the wall for each bed. When we first visited the facility, the rooms seemed too crammed. And yes, if somebody is agitated and screaming you will hear them scream. However, the common areas are very nice and spacious. Most people hang out in the activity room, dining room or in the hallways during the day, so the residents’ rooms are only used when the resident is in bed. Everything is VERY well maintained. Walls are painted and have art work on them, common areas and the lobby have nice furniture and nice décor. The place is clean, there isn’t any odor like so many other places have. Even the facility’s façade is painted in fresh bright colors. Lastly, there are few outdoor areas with nice landscaping and patio furniture, it definitely is nice to get outside when we visit.
In summary, out of the places that accept insurance that we visited, this one is by far the best for long term care. I am very happy with the care my mom receives, I trust this place, and have a piece of mind. If you are looking for a place for your loved one, I am sorry you are in this position, but I would definitely recommend Valley house.

My husband is in Valley House for the 3^rd time. He is now there for physical therapy.
I must say when I first came I was a bit apprehensive being that this was our first stint at a rehabilitation center. All my anxieties were put aside by talking to Lorna and learning that any concerns or issues are to be brought to managements attention and any adjustments that are required would be made right away.
I am very happy that the nurses and staff remember my husband and treat him with respect and kindness. This type of treatment helps the healing process. I feel quite confident that my husband will be up and walking on his own very soon. I must mention one of the CNA’s «JOHN» that young man is the kindest, considerate and loving individual I’ve ever known, no mater what the task he does it with a smile and a kind word; with people like him working at Valley House all the patients will thrive and get better sooner than later!
As with any business you cannot please all the people all the time but I see that Valley House is really trying to do a good job and I appreciate that immensely!

My grandmother was brought to this facility to do some rehab so she can be stronger before she goes home. The goal was to strengthen her body so she would be able to walk without assistance. 3 weeks later she passed away.
Sure you may say how is her death had anything to do with her stay at Valley House.
No, I’m not suggesting that there were any foul play involved in her death.
What I’m saying is that a series of events that transpired eventually caused her death. For instance, she was barely eating and eventually stopped eating but Valley House never notified our family of this. As a matter of fact, we were the one that informed them of this. Their doctor never checked out my grandmother during this time nor did they take any action to send her to the hospital until a member of the family told them to do so. There was no DNR (Do Not Resuscitate) or advanced directive at this point which means they needed to do all means necessary to keep her alive and her health from failing. It was quite obvious that she was losing a lot of weight but not a peep came from the facility. She had lost around 20 pounds during her 3 week stay at Valley House. During my grandmother’s last days, the nurses said that they were giving her her medications and that she was taking it in. This is quite suspicious since she had stopped swallowing about 4 days prior to her passing. It only means that they are not thoroughly checking their patients whether or not they are actually ingesting their medications.
When my grandmother passed, the doctor, Tam Nguyen, would not sign her reason for death and death certificate form. She had been dead for a week but Tam Nguyen kept coming up with various excuses and lies on why he didn’t sign the document. He said he was out of town and just got back, he never received the documents, he received the death certificate but with no name on it, he doesn’t have a fax machine, and so on. This doctor is a liar and very arrogant and does not care about any of his patients nor their grieving families. We almost didn’t get to bury my grandmother. He finally signed the death certificate just 2 hours before her burial. He had put our family in further anguish when we were already grieving but he didn’t seem to have any care whatsoever.
The facility didn’t offer any condolences or sympathies for our loss. Although some employees offered their sympathies but none came officially from Valley House. Valley House seem to have treated my grandmother as just a number and didn’t care about her as a person, especially as a person that people loved and cared for.
This place is horrible and should not be trusted with your love one.

When my mom was in 209 with Jenny, Dianne and Rochel had a conference with me and agree that Jenny can only watch TV until 10 pm, and after 10 pm if she want to watch on, she had to put on the headphone. But then that time, Tina stop me and told me that there was no such rule. She was flip-​flopping and went against the agreement. And a lot of time my mom was cold because the window on Jenny side was open late at night or overnite, and Tina stop me again and said «the window was on her side». And now that my mom had moved to 212, and for some reason the room was hot (even Myrna said that one time) and she want to open the window but her roomate didn’t like it. And do you know «who» stopped me again? You’re right! It’s Tina again! And this time another flip-​flopper again. She said my mom could not open the window because the roomate lady didn’t like it. She is a «licensed» professional and she has violated the law. Her license should be revoked and she should go to jail. This is «professional malpractice». When I told Lorna (The D.O.N) about Tina’s flip-​flopping attitude about the window, Lorna said «whoever complain has to move». So, I turn the table on her and ask her «so, this time the roomate lady complain, does it mean she has to move?» Lorna realized the argument was turn against her so she had to shut up. And Joy, the staff development director, was there also and could witness about that. The administrator, Merlin, didn’t allow me to sleep while sitting in my mom’s room.(And Stephanie [MDS] and a black social worker were sitting there and nodded their head in approval of that. And Maria [the dietary supervisor] was pretending going in and out of the room to check some folders something to cause some distraction) And now a Licensing and Certification evaluator ask him if that was a problem, he said «it was not an issue». Obviously, he a was attacking me personally that time and I believe — as a licensed professional — he has violated the law. So, I think his license should be revoked and he even has to go to jail. (After the forbidding, one time I was so tired and lean against my chair and sleep a little bit, but Valley House contacted my brother in Florida, and he text me and told me that they told him that I was still sleeping in there and that I could not do that again or they will forbid me to come to visit my mom again. After that I could not rest at all. I had to log on Youtube with my phone and listen to one song after another. My ears was like being bombarded by the music — which is something we usually enjoy. I couldn’t stand it any more and had to ask my dad to come and replace me to care for mom) Another murderous thing about Merlin is that sometime ago the red curb on the right side of the nursing home (from the inside facing out) was cut short by the city, and I ask the Valley House Staff to contact the police or the city to restore it to its original length because it was really dangerous. Several times, when I was driving out of the nursing home, I had to come like about the middle of the street in order to have a full view of the traffic coming from the left. And two or three times I was almost hit by a car coming by. One time, a visitor was driving out and he couldn’t see me and came out about the middle of the street, so I had to stop in the middle of the street to let him out completely. One time, Preet (the director of the activities department[maybe came back from her lunch]was driving quite fast from the ouside toward the entrance, so I had to stop at the entrance to wait for her to get into the parking lot before I could drive my car out. So, the situation was really dangerous: to the residents, the visitors, and also the employees. So, I — a visitor and a resident of San Jose — had to call Santa Clara police and the city of Santa Clara to ask them to correct that dangerous situation. Finally they did that, but only after my several phone calls, and a lot of persuasion, and a long waiting period of time. And on the nursing home’s side, they not only refuse to do that but also –that time– Stacy Huynh (the accountant and billing agent) even yell out loud at me right at the receptionist window. And Merlin intentionally (I believe) open his office’s door (which is just a few feet away from the window) and so, was kind of like telling me that he heard all of the conversation and he backed what Stacy said and even her attitude (yelling at me).That was unbelievable and totally unacceptable. He was putting the lives of the residents, visitors, and the employees at risk. The situation was extremely dangerous: for the residents, visitors, and also for the employees, but he refused to do anything about it. So — as the administrator and a licensed professional — I think he shoud quit his job and let somebody else who is more professional and conscientious to take over. I think the Colkitts should force him out even if he doesn’t want to go! He is not fit to be the administrator of this nursing home

I transferred my mother here after it was recommended by her Assisted Living facility.
What a mistake. I don’t know what they were thinking as my mom’s room was at the end of a long crowded hallway, directly opposite a screaming woman in restraints who was refusing to take medicine.
A group of wheel chaired residents also clustered together in the hallway right outside my mother’s door. Being older, they spoke loudly in an effort to hear each other over the din of the screaming.
The night my mom was admitted, she was given «dinner.» It consisted of a limp, microwaved tamale and some watery, over-​cooked green beans. Her room was tiny, shared, and well-​worn. The staff at the desks were stoic and unfriendly. As I roamed the hallways, it felt crowded and hectic. As I left her that night, I was in tears. I was up at 5am the next morning, searching other facilities. By 10am, I moved my mother into a different place.

Anyone who has been to as many Rehab Centers as I have (due to many misfortunes with my parents), would absolutely agree that this place deserves no less than 5 stars. Nobody likes hospitals or rehab centers, as they are usually places of great sadness. But Valley House is the best I’ve experienced in the Bay Area. Top reasons why:
1. Clean — this place is very, very clean for the type of business it operates. They are constantly cleaning the rooms, the patients and the common areas. This is the only Center that does not smell… it was the first thing my sister and I noticed when picking it out.
2. Nice & Attentive Staff — staff members at Rehab Centers are always very busy and quite stressed out. But the people at this Center are very friendly, patient, attentive and usually smiling. Not only does this brighten the mood of the patients, but it provides an uplifting environment for everyone, including the visitors.
3. Good food — rehab centers don’t usually provide the best food, but the food at this place is actually quite good! Since his stay (my father has been here for 2 weeks as of today), my Dad has had filet mignon, chicken, salmon, pasta, veggies, fresh fruit, and much much more. My Dad cleans his plate every meal :)
4. Nice Facilities — the building and furnishings of this rehab center is the nicest I’ve seen. It is quite modern, and has nice paintings and art in every room. The patients’ rooms are very pleasant, and equipped with dressers, night stand, and a TV with cable and DVD player.
5. WiFI — free and speedy WiFi! Need I say more?

I had the pleasure to stay at Valley House for both my knee surgeries., in two different occasions.
From the top administration, social workers and a wholesome staff took care of my recovery with so much compassion and expertise.
I could notice that they are in fact making upgrades of the building.
I would like to go into detail:
The whole department of PT and OT lead by Kim have not only professionalism and respect but their techniques are individualized and one on one, they get to learn about you and what will be your challenges upon coming back home. Thank you Poorvi, Long, Raddy, et al.
The whole staff of nurses and assistants made my stay comfortable and their dedication is way above the call of duty. I could mention some names this time around: a Trinity, Tina, Virginia, Anna, Doreen, Clifford, Bertha — again an example of dedication is giving me the medicine with apple sauce so I could swallow better. Hard working to the max.
Their social activities are well prepared and entertaining!
The place has a great atmosphere and is kept clean and sanitized!!! Thank you guys for being so attentive with myself and my family. God Bless You

I was there visiting my dad and I found out that they didn’t take my dad to the bathroom cuz my dad told me that he has been asking many times before the dinner starts, but nobody care about him. So he was eating his dinner with wet pants. So I told the nurse then she took my dad to change right away. I have feeling that people work here are just doing more their jobs when someone is visiting. when I was there I also hared someone was callling for nurse to help but nobody comes so she was keeping push the bell. I think they acted really care about the residents in front of visitors but according to my dad he always has to tell them many times. My dad doesn’t have Demancia or Alzheimer. He just has a problem with his legs. Anyways I am thinking of hiring someone to take care of my dad at my house or find some other facilities. I don’t care about the other things that much, but the only thing I wish is when someone I really care needs some help I want them to help him. Anyways, I am looking for someone who I can really rely on not just in front of us.

My Grandfather had an amazing experience. Staff was very friendly and he enjoyed his stay. The therapists were very motivating to their patients. He had severe gout that he couldn’t walk. The therapists did an awsome job getting him back to walk again. My Grandfather is a very particular man. At first I was afraid he was going to complain about the facility but overall I never heard a complain.

Please check their recent state reviews, their inspections (they are required to show you if you ask) and review their deficiencies prior to staying there or placing a loved one there. Also call the state agencies and check on their complaints as well. I know they have a lovely young lady doing commercials but after seeing them and seeing the marketing they are doing. I myself had a very very unpleasant stay there asked to be moved and felt compelled to speak up. I have photos as well and will try to post them. Often family members visit during the day and the entire night staff is different. Please visit at different times of the day and night examine the bathrooms see actual rooms and see the loudness factor and the way staff treats the patients and their cleanliness and odor as well. Talk to the evening staff, ask about the way they handle evening patients and their cleanliness and hand washing standards. I would do your homework before accepting treatment or placing a loved one there. I do hope Unilocal posts this because when I did research I did not see any feedback reflecting the true patient care that happens here.

Excellent, friendly staff, particularly David, Jono, Anne, and the midnight shift CNAs.. I came in after an operation, working to get on my feet, and the CNAs and therapy staff were totally supportive and pushed just the right amount to get me out when I wanted. When I got home I was confident in my ability to function and have done really well so far. I felt like the nurses were pretty good, not always as timely as I would have liked. The food — well it is institutional, enough said. Air conditioning is not the most effective. The staff though — they all made me feel like their favorite patient ever.

WOW!!! «WHAT A GREAT VACATION!!!» I honestly didn’t want to go home. They made me feel like I was the most important person there. Here is a typical day at my favorite resort in San Jose.
7−8am I was awaken by a smell of a delicious breakfast; hot coffee, eggs, sausage, fresh strawberries, pancakes, or French toast. They have this amazing fat free, sugar free syrup I put on almost everything.
9−10am After breakfast and watching the news on my TV, I would get myself dressed, or call for assistance, to get ready for MY PERSONAL daily physical therapy session. It was about an hour and a half long. There are two kinds of therapy, occupational and physical. Occupational is about learning how to do things around the house so you will be safe, like getting in and out of bed. Bending over the correct way, and dressing yourself. Physical is strengthening your body to be strong again to do the things you love to do. The therapists are wonderful. They’re patient, helpful, smart, funny, and compassionate.
12−1pm Lunch time! I would be back in my room. Every room has a partition between the beds so it very private. I had a great roommate so it wasn’t a big deal. There’s a sliding glass door that opens to a patio, or grass so you can let the fresh air and the breeze in. Lunch would be served around this time. Again the food was always prepared to your specifications. Maria is the chef. I was amazed on how personal she was at making sure I got everything I asked for. Again, treating me like I was most important person there.
1−5pm FREE TIME!!! to do anything you want. You can take a nap, watch TV, go bug the staff, go on their computer, look for the snack cart that rolls around every day with tea, juice, cookies, cheese, and crackers. The gals who run this are so sweet, and there is no limit!!! There are beautiful grounds to go sit outside in the sun and their lobby is very comfortable too!
5−6pm It’s time for dinner. If you’re not in your room, when it arrives they will leave it on your bed tray, and when you return, just buzz them and they will come and heat it up for you with a smile.
6−9pm After dinner you could go watch the movie of the day, or get ready for bed and relax. Dessert was also offered every night around 8pm.
I could go on and on about how great ALL the nurses are but it think you get the point that it’s a pretty nice place to recover. Also your pets can come visit you, that always makes you feel better!!! I hope this letter will help you make the right choice on where to stay.

This place is awesome! It is run like a clock by Mr. Davey, and the Therapists are the greatest! They helped my Mom recover from a stroke, and they were VERY impressive, this was the most important thing to me, I wanted my Mom to get better and they made it happen, I would HIGHLY recommend this place to anyone you care about that needs to go to a re-​hab, the nurses were great also and I was impressed about there knowledge and care, like I said I thought it was run very smotthly and everyone had a job to do and did it well!

This facility is very clean and the nurse staff is so nice. The furniture in the lobby does seem a bit old school but its very comfortable. The rooms are shared and that alone makes the room a bit cramped but at the same time my grandpa still has his privacy. I have been to visit my grandfather on two occasions and he is quite happy. I keep asking him how they are treating him he said good. He likes the nurses and from what I observed today they enjoy his humor and the fact that he doesn’t complain. My aunt visits him daily and has gotten to know most of the staff, she said they have all been very friendly and helpful with any and all requests. I would recommend this facility if you are looking for somewhere to have your loved one.

Ok 1 stars for the facility and 1 stars for the staff, therefore making this place 2 stars.
The facility is dated. They need some major remodeling. I mean MAJOR! I feel like I’m entering into a time warp whenever I pull up into the parking lot. Hella late 70’s décor, but then again maybe it’s like that to make the residence feel like they are back in time or at home.
The rooms are small. Actually pretty cramped. My dad’s space seems like he is shoved in a corner making the room so tiny. Granted he shares a room, but still. His space seems very small.
Enough about the facility. The positive thing about this place is that the staff is friendly. I know nursing homes/​skilled nursing facility places are sometimes not so «visitor friendly» because most of the times the visitors complain about something not being done, etc. One of the nurses that I encountered named Anna was very helpful, attentive, and friendly. She was hospitable as well making sure my visit was pleasant. Actually she was the only night nurse who I thought was helpful and friendly. The others were kind of just like, «oh… it’s another new patient,» kind of attitude.
I just think that the staff should be a little more attentive to the patients. I know it’s hard when the patient to nurse ratio doesn’t accomodate the patients needs. I mean if I was the nurse on duty, I would be checking in every 30 mins or so to make sure the patient is turned over or if the patient needs a diaper changed. But then again, that’s just me and I don’t like being watched let alone evaluated by a visitor.

My step-​father has been in rehab for over a month now, and we’ve had the great fortune to find Valley House.
In a nutshell, Valley House is fabulous. It’s a pretty place, to be sure. It’s truly the dedication of the wonderful people who work there that sets this place apart from the others. Everyone we’ve worked with — Administrators, Nursing Staff, Social Workers — they all have the same cheery and supporting attitude that it takes to make a place like this work. Many patients are demanding and unhappy, and they have a right to be as life has dealt them some tough cards. The amazing employees at Valley House have never-​ending patience and concern for the people in their care and this is truly something to marvel at. (Trust me on this, I spend a few hours there each week and my patience is worn thin by the time I’m heading home.) The Valley House staff is respectful and careful with their patients.
You have to see and experience these other places to truly appreciate all that Valley House has to offer. Many of these types of facilities are smelly, dismal, understaffed places that don’t foster a «we want you to get well» attitude. It’s hard to believe that people actually get better in those places. (My step-​father had been in one such place before we found Valley House — check my next review.) Valley House is a refreshing change in philosophy when it comes to longer term care.
BTW, I contacted our local VA office to ask for help in finding a good place (after one horrible experience we were looking for help). In the words of the person there at the VA, «We LOVE Valley House!»

Valley House is the nicest n most well kept nursing homes that I’ve been too.
Most of their staffs are friendly n helpful.
I feel that their AM staffs are much better than their PM ones. After spending most of my days here for almost a wk, I feel like the AM nurses listens n delivers requests in a timely manor. Mayb the AM staffs doesn’t have as much things to do than the PM staffs or watever their reason might b.
Overall I give Valley House a 4 star bcuz I felt like there was a couple of PM staff that didn’t care as much. But overall I like Valley House n would recommend this facility to others.

I visited Valley House Rehab Center recently and got a quick tour of the place. I was very impressed: this is not a typical hospital. I mean, you can tell that it is a hospital, but in some ways it’s more like a resort. They seem to try really hard to take care of their patients, and keep them happy.
They have a Paris-​trained chef who cooks gourmet meals for the patients. I was very surprised by this, since my own experience with the food at other hospitals has not exactly been the greatest. I was told that the food is not only delicious but is healthy. There is also a «welcome social dinner» each month for the patients and their families. There are all kinds of interesting & fun activities, and even some church services. There is even a massage therapist. Very impressive!

After I transfered my father to Valley House from Mission De La Casa, I felt so good, and relaxed. The facility is so clean, bright through out. The rooms, bathroom are spotless and get cleaned regularly. The staff are so helpful, and friendly, and very competent. Their physical therapy room is equipped with all the needed equipment. and the therapists are so good at what they do with the residents. Every room comes with flat screen TV and cable hook-​up, hospital beds. Nice, big Dining room. I highly recommend this nursing home! Valley House Keep Up the Good work! A+++
