History

Established in 2015.

Dr. Carlson has an extensive medical background. He achieved his undergraduate degree at Butler University, Indianapolis Indiana and post graduate studies in neuroanatomy at the University of Florida. He completed his physician assistant training with Bachelor of Science in Medicine at the University of Florida. After 17 years in cardiovascular medicine and achieving the 1994 physician assistant of the year award, Dr. Carlson received his Doctor of Medicine degree. He completed the Bayfront Medical Center Family Practice Residency Program and was the only second physician to receive the 2003 patient excellent award voted by the nursing staff.

Dr.Carlson started with Palms Primary Care 09/​01/​2015. As a new provider with HCA he is committed to the personalized individual care for the medical community, his patients and families in the Tampa Bay area.

Danielle graduated from the University of South Florida with a bachelors in Physiology and a minor in Biology, and is currently pursuing her masters in Healthcare administration.

Danielle has worked in the medical field for more than 10 years, and is dedicated to the patients, doctors, and employees of her practice.
